I think the rarest way to die is death.fell.accident.water but not the kind you set up with a minecart and stuff. I once had a hardcore world and spawned on an island. I then entered the room with the iron and stuff in it in a shipwreck. When I was leaving through the window, I instantly died, and the death message was death.fell.accident.water
