I cracked the shard and read it, didn't bug. My first try I broke it, I was double-jumping from the tall building and scanned everything from above.
Might be the scanning sequence and Johnny's voice lines. I had another quest bugged that involves Johnny appearing and making comments but I screwed up the sequence.

I did this misson 9 times got 3 different bugs XD qualitycontrol at cdprojectred
SOLUTION: You do everything he does.. except once you got to the guy where johnny says "grirny (?)" and she appears in the tub, you walk to the guy with the splinter loot it and instant decode it.. this is the only way 0 bugs appeared for me ^^ patch 1.06
