  1. There is a game over in god of war 2 that fits this list. During your fight with the sisters of fate, you get taken back to your fight with Ares in the first game, one of the sisters (I can't remember which one) tries to break the sword you used to kill him. If she succeeds, you get to watch both gow1 Kratos get brutally impaled by Ares's sword, and gow 2 Kratos die of unseen injuries.
    Edit: there is also an ending in the gow 1 Ares fight, partial way through, he opens a portal where you see your wife and daughter (for context, Ares tricked Kratos into killing his wife and daughter before the game started) and you have to defend them from clones of Kratos. If you fail, they die, and Kratos drops to his knees, then gets beaten to death by an army of Kratos clones.

The Wing Commanders games also have a unique Game Over screen where if you fail objectives or break the rules too often the protagonist Christopher Blair played by Mark Hamill is seen in an FMV cutscene court marshaled and dishonorably discharged. We then see Mark Hamill solemnly sit and drink (presumably himself to death.).

  3. As dark as that ending is for the Phoenix Wright series, I still find it ironic you mates chose possibly the most light-hearted bad ending in the series. Yeah, LIGHT-hearted bad ending. I say Spirit of Justice and Apollo Justice's bad endings were even darker. Spoilers for both games below.

    First: Spirit of Justice's bad ending would lead into not only the corrupt reign of Queen Ga'ran continuing, but also the execution of both Phoenix Wright and Apollo Justice challenging her position. Pretty dark, but Apollo Justice's bad ending is even darker in my opinion.

    Apollo Justice's bad ending (and an official bad ending as opposed to the theoretical one for Spirit of Justice) involves the choice of voting Vera Misham "Guilty" in the jurist system trial. Voting her guilty pushes back the final verdict by one day. But in that one day, due to the poison she accidentally consumed previously while in court, her condition worsens, causing her to die. The final verdict would never be reached, Phoenix Wright wouldn't get his badge back, The Dark Age of the Law would continue on indefinitely in Phoenix's land, and the events of Spirit of Justice's bad endings would be all the same except Phoenix and Apollo aren't executed.

  5. S.O.S./Septentrion for the SNES. The ship you're on starts to sink completely when you have around a minute left on the clock, and you're left frantically searching for a way out as the water rises and grim music plays in the background.

  6. The only thing in my perspective that I wouldn't recover from is seeing that body in the wasteland from Fallout 2.
    Edit: What even creeped me out more was that I was watching this at 3 AM MDT
    Also Edit: The dialogue starting from "You died" made it creepy as frick
    Edit (Again): Ok now at this point I'm replaying this exact part because apparently I have an urge to look at pictures like that while a fail dialogue is playing
