I think you turned atheistic because you picked the irreligion culture. Which was kind of odd given all the prior decisions with the olmec heads and celtic trees.
Also I think the way it works with the war demands is that whatever grievances you used to pushed to start the war, those are automatically part of the peace deal which is why they were automatically checked. The other provinces have to be demanded separately. And if a city has multiple provinces, they all show up with the same name. But as you saw, if you leave one out, you’ll get the main city and any additional attached province you asked for but the one that wasn’t selected will break off as its own city and remain with the defeated faction.
You can ransack your own tiles, including your own cities though. So if you want an outpost instead, just burn the city to the ground and claim the land.
Just like how your troops may raze your own districts to the ground, if you wish to purge the city you can ransack it with your troops; your troop can also set up an outpost immediately after the plunder. This is way better than Civ's option of immediately raze or permanently keep – since you can choose to pillage the city when the time arises – rather to lose it to warring empires or to use scorched earth tactic.
