This video shows the Romance with Shadowheart in the early access of Baldur’s Gate 3.
To be able to romace and kiss Shadowheart you need atleast ‘Medium’ Approval rating. You will egt the opportunity to kiss her in the campsite after rescuing the Druid Halsin and finishing the side quest ‘Save the Refugees’.
In the Early access you do not get to have sex with shadowheart but by the hints it seems like it might be possible when the full game releases.
