  1. A Neothelid would be an interesting super boss.
    Sorta akin to Dark Souls 3's Midir or Pokèmon Soul Silver/Heart Gold's Red.
    Something tucked away, hidden and optional.
    I'm sure it'd drop decent loot but the allure for me would be the fight itself.

  2. It should die and shrivel up though

    Only the largest tadpoles in the brine pools start to cannibalise each other as they starve. They naturally grow, however if they become too large they would normally be either killed by a monitoring mind flayer or fed to an elder brain 🧠

    The largest tadpole left alive in the pool would grow into a neothelid and become a mindless killing machine with its hunger for intelligence and brain matter leading its now primal urges.

    It is said that one thing Illithid/mind flayers fear are Neothelid’s as they’re intelligence is so low, that mind flayers cannot detect their thoughts to locate them and can unknowingly be hunted by them.

  3. If the tadpole comes out when the host dies, then in order to get rid of it, one simply needs to die and then wait until the tadpole comes out, and then someone in the party can use a scroll or pay 200 to the undead dude in the camp to resurrect the dead companion, problem solved!
